To register for the organ donor registry online, click "REGISTER NOW!" on the right side of this page. Select the state where you live, fill out the online form (make sure you have your driver's license handy!), and follow the steps to becoming a miracle maker!



 

FIRST PERSON CONSENT

Does your state registry say that it is a first person consent registry? This means that for anyone age 18 and older, the decision to donate is legally binding, ensuring that your organs are donated and noone, except for you, can reverse that decision.

 

SAY "YES!"

Not so computer saavy? Not sold on signing up for the organ donor registry today? Say "YES!" next time you renew your driver's license and they ask if you woud like to be an organ donor. Put that little red heart on your driver's license!

 

I AM A DONOR!

If you have signed up on your state's organ donor registry, THANK YOU! To ensure your wish to save others is carried out, make sure to put the little red heart on your driver's license to designate your decision, inform your family of your donation decision, and include it in any advanced directives or wills.

MAIL-IN FORM

Prefer paper and pen? Contact your local DMV (you can find your state's phone number at www.dmv.org) and request an organ donor registration form by mail. Simply fill out the form, send it in, and you're done! If you live in Kansas, call (888) 744-4531 to obtain a copy of this mail-in form.​
